Registration
All Lakeview Medical Center patients will need to register at the Admission Office located just inside the front entrance. The information we require is essential to provide proper care for you. Also, government regulations require that we obtain certain information.
It is important to bring with you any identification cards or documents pertaining to insurance coverage. Please present them on the day of your admission, if you did not already do so at the time of pre-admission registration.
At the end of your admission, you will be given an identification wristband. That band identifies you to each person who participates in your care and must be worn throughout the duration of your stay.
If you have pre-registered, please stop by the main desk to announce your arrival. If you are coming to the hospital after hours, please register at the Emergency Room desk, located inside the front entrance to the right. Please have your insurance information with you when you register.
After you have registered or checked in at the main desk, you will be directed to the appropriate department. Please have a listing of all prescriptions and over the counter medications you are taking with you.
All patients are advised not to keep any cash, valuables or jewelry with them during their stay.
Pre-Admission Registration
Some patients may find it more convenient to pre-register for an upcoming hospital stay or procedure. Some insurance companies may require you pre-register. Click here for a Registration form you can fill out and bring with you to registration.
When pre-registering, please have your
