Outpatient Social Worker

Position Description
The Social Worker provides
psychosocial evaluation, counseling and resource referral
services to patients and as needed with families, works in
collaboration with the multidisciplinary treatment team to
incorporate psychosocial needs and concerns in planning
care. This position will work with the LMC Dialysis Units
in Rice Lake and in Spooner, WI.
Qualifications
Must have a Master’s in Social Work and State of Wisconsin
license or certification. Minimum of two years of clinical
Social Work experience, preferably in a medical setting.
Experience preferred in dialysis. Must have a valid driver’s
license and be able to provide own transportation.
Normal Work Schedule
Hours are
flexible for half days Monday thru Friday or full days a couple
days during the week. Weekend call is every 4th
weekend from 8am to Noon by telephone.
