Pool – Casual Status – must be able to fill in as needed. Minimum of 2 shifts per month.

Position Description:
Provides care in patient’s home and/or, for hospice patients, in the inpatient/respite setting of Lakeview Medical Center, in accordance with program standards and under the supervision and instruction of a registered nurse.  Completes assigned patient specific duties for patients.  Included in this responsibility is accurate record keeping, documentation of the duties completed and notification to the RN of any change in status observed on a visit to a patient.

Qualifications:
Completion of a competency evaluation program and listed on the State of Wisconsin Registry as eligible to be employed at a state and federally-certified Home Health Care/Hospice agency.  Previous hospital or nursing home experience preferred.

Normal Work Schedule:
Flexible daytime hours.  Must be willing to travel within a 40 mile radius of LMC.
